Job Description:

Divine HealthCare Services LLC is a home health provider with a branch office located in Richmond, VA. Currently seeking an experienced, compassionate and skilled LPN to join our Home Health Agency team. The LPN will provide high-quality, patient-centered nursing care to clients in their homes, ensuring comfort, safety, and continuity of care. This role requires strong clinical skills, excellent communication, and a passion for serving individuals in their home environment.

Key Responsibilities:

Perform comprehensive nursing assessments in patients’ homes.
Develop, implement, and update individualized care plans.
Monitor patient progress and document care accurately and timely.
Educate patients and families on care plans.
Coordinate care with physicians, therapists, and other healthcare team members.
Ensure compliance with state, federal, and agency regulations.
Participate in case conferences and care coordination activities.

Qualifications:

Current LPN license in the state of practice.
Minimum 1 year of nursing experience (home health experience preferred).
Strong clinical assessment and critical-thinking skills.
Ability to work independently and manage time effectively.
Excellent communication and documentation skills.
Valid driver’s license and reliable transportation.
Current CPR certification.
Ability to pass background check and drug screening.
Experience with OASIS documentation.
Knowledge of Medicare/Medicaid home health guidelines.
Familiarity with electronic health records (EHR).
